We know life can be busy, so we have a wide range of appointment times available, including two days a week where we work evenings.  If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible. We do ask that you provide us with at least 1 business day's notice for appointment changes.

Extended Health Insurance is a contract between the patient and the respective insurance company. We will provide you with a statement that you can submit to your insurance company.  We are able to electronically file for some traditional insurance plans, contact our office to verify acceptance of your plan.

Monday: 9:00 am - 5:00 pm (Closed for Lunch 12:00 - 2:00 pm)

Tuesday: 2:00 pm - 8:00 pm

Wednesday: 9:00 am - 5:00 pm (Closed for Lunch 12:00 - 2:00 pm)

Thursday: 2:00 pm - 8:00 pm

Friday: 9:00 am - 3:00 pm
